Can You Tighten Loose Eyelid Skin Without Surgery in Montreal?
Loose, crepey skin around the eyes can often improve without surgery. Compare Sofwave, Fotona, CO₂, RF and TriLift, plus the combinations that fit each concern.

Key facts
- Sofwave's manufacturer describes a typical protocol of 1 to 2 treatments, depending on skin condition and clinical recommendation.
- Fotona currently recommends at least 4 SmoothEye sessions for optimal periocular treatment results.
- A published fractional CO₂ eyelid study used 2 to 3 sessions and reported improvement in eyelid tightening one year after treatment.
- Lumenis recommends a course of 4 to 6 weekly TriLift treatments.
Yes. Mild to moderate loose or crepey skin around the eyes can often be improved without surgery. Depending on the concern, treatment may involve 1 to 2 Sofwave sessions, at least 4 Fotona SmoothEye sessions, or 2 to 3 CO₂ treatments.
The right option depends on whether the main issue is a lowered brow, fine crepey texture, deeper wrinkles, loss of skin firmness or changes in the tissues supporting the eye area. There is no single device that treats every cause of an aging eyelid.
Why does the skin around my eyes become loose and crepey?
The skin around the eyes is thin and constantly moving. With age, the entire periorbital area changes, not just the surface of the skin. Collagen and elasticity decrease, while changes also occur in facial fat, muscle, fascia and the bony structures around the eyes. Chronic sun exposure adds further collagen damage and contributes to wrinkles, loss of elasticity and uneven texture.
This is why two people who both say, “my eyelids look loose,” may need very different plans.
One person may mainly have fine crepey texture. Another may have brow descent that makes the upper eyelid appear heavier. Another may have true excess eyelid skin, under-eye hollowing or a drooping eyelid margin.
For mild to moderate skin laxity and wrinkles, non-surgical technologies can often improve the appearance. For substantial excess skin or true eyelid ptosis, energy-based treatment cannot physically remove the excess tissue or repair the structures that elevate the eyelid. Those cases may require assessment for surgical treatment.
Can Sofwave create a non-surgical eyebrow and eyelid lift?
Sofwave is particularly useful when brow position and skin laxity are part of the problem. Its ultrasound technology delivers controlled energy beneath the skin surface to stimulate collagen remodeling.
Sofwave has a specific cleared indication for eyebrow lifting, in addition to improving facial lines and wrinkles. Its smaller Precise applicator was developed for curved areas such as around the eyes and mouth.
At Ideal Body Clinic, Sofwave skin tightening may therefore be considered when the goal is to improve mild laxity around the brow and upper eye area without resurfacing the skin.
The manufacturer's patient protocol generally describes 1 to 2 treatments, depending on the patient's skin condition and clinical recommendation. Collagen remodeling develops gradually after treatment.
Sofwave does not remove a fold of excess eyelid skin. Think of it as a collagen-remodeling and lifting treatment rather than a substitute for surgical blepharoplasty in someone with significant dermatochalasis.
Is Fotona SmoothEye better for crepey eyelid skin?
Fotona SmoothEye is designed specifically for the periocular area. It uses non-ablative 2940 nm Er energy in Fotona's SMOOTH mode to heat tissue in a controlled way and stimulate collagen remodeling without ablating the surface like a traditional resurfacing laser.
This makes Fotona laser treatment around the eyes an option when the concerns include:
- fine eyelid wrinkles
- early crepey skin
- mild laxity
- reduced elasticity
- upper and lower eye-area texture
Fotona's current patient information recommends a series of at least 4 SmoothEye sessions for optimal results. A prospective clinical study using the same 2940 nm non-ablative Er approach treated patients with 3 sessions spaced 4 weeks apart and reported improvement in periorbital wrinkles and laxity.
Fotona is therefore useful when someone wants gradual tightening with less surface disruption than an ablative CO₂ resurfacing treatment.
When does CO₂ laser make more sense around the eyes?
When the dominant problem is visible crepiness, etched lines, sun damage or surface texture, fractional CO₂ resurfacing can provide a stronger resurfacing approach.
Published studies of fractional CO₂ around the eyes have reported improvement in periorbital wrinkles, skin texture and eyelid tightening. One study treated the upper eyelids, lower eyelids and surrounding eye area with 2 to 3 treatments. Another prospective study used between 1 and 4 treatments depending on the patient and treatment plan.
At Ideal Body Clinic, Secret PRO CO₂ and RF microneedling allows treatment planning that can address both surface resurfacing and deeper dermal remodeling. Cutera lists fine lines, aging skin, sun damage, tone, texture and overall skin quality among Secret PRO's applications.
CO₂ generally involves more recovery than non-ablative options. That tradeoff can make sense when surface wrinkles and texture are the main concern rather than mild laxity alone.
What can radiofrequency do for the eye area?
Radiofrequency treatments use controlled thermal energy to heat the dermis and support collagen remodeling. Reviews of RF treatments describe improvement in skin laxity and wrinkles, including in the periorbital area, although results and protocols vary substantially among devices.
RF can therefore fit between very gentle treatments and ablative resurfacing.
For someone with crepey skin and mild laxity, RF may be used as part of a plan to improve dermal firmness. RF microneedling can deliver energy at controlled depths for more intensive remodeling, while non-needle RF treatments focus on heating without puncturing the surface.
There is no universal number of RF sessions because different RF systems, depths and energies are not interchangeable.
How does TriLift help around the eyes?
TriLift approaches facial aging differently because it does not focus only on the skin.
The platform combines Dynamic Muscle Stimulation with TriPollar RF and RF microneedling options. Lumenis describes its muscle stimulation as targeting the cheek, jawline and under-eye regions to condition healthy facial muscles while RF technologies address the skin.
That makes TriLift facial toning interesting when the goal is not only smoother skin, but also support from the surrounding facial musculature and midface.
The manufacturer recommends a course of 4 to 6 weekly treatments.
TriLift should not be thought of as a CO₂ replacement. CO₂ primarily resurfaces visible skin texture. TriLift adds a muscle-conditioning and RF component. These can address different parts of the aging process.
Which combination makes the most sense for my eyes?
There is no single “best” combination for everyone. A better approach is to match each technology to the problem it is intended to address.
Main concern
Treatment approach that may be considered
Mild brow descent with early laxity
Sofwave, with Fotona SmoothEye if fine texture is also present
Crepey eyelid skin with fine wrinkles
Fotona SmoothEye, or CO₂ when stronger resurfacing is appropriate
Deeper lines and visible sun-damaged texture
Fractional CO₂, sometimes followed by a collagen-maintenance treatment
Mild laxity with little desire for downtime
Sofwave, Fotona or non-ablative RF
Texture plus deeper dermal laxity
CO₂ resurfacing with an RF-based treatment plan
Eye-area aging plus loss of facial muscle support
TriLift combined with a skin-focused treatment such as Fotona or Sofwave
The exact Ideal Body Clinic combinations have not all been compared directly against each other in randomized trials. The reason for combining them is to treat different layers and different problems, not simply to perform more procedures.
Can Sofwave and Fotona be combined?
Yes, they can be considered as complementary treatments when both lifting and eyelid texture are concerns.
Sofwave is more focused on non-invasive ultrasound-based collagen remodeling and eyebrow lifting. Fotona SmoothEye is a periocular Er treatment focused on wrinkles, elasticity and tightening.
A staged Sofwave plus Fotona SmoothEye plan can therefore make sense for someone with an early heavy-brow appearance together with fine, crepey eyelid skin.
The treatments do not necessarily need to be performed during the same appointment. Timing should be determined after assessing skin condition, previous treatments and recovery.
Can Sofwave be combined with CO₂?
For someone with both laxity and substantial surface texture, Sofwave and CO₂ address different targets.
Sofwave can be used as the lifting component of the plan, while CO₂ can address fine lines, crepiness and photodamaged surface texture. Published reviews of periorbital rejuvenation support tailoring different energy-based modalities to the individual anatomy and aesthetic goal rather than relying on one treatment for every problem.
Because CO₂ creates a healing response in the skin, the treatments may be staged instead of automatically stacked in one visit.
Can TriLift be added to an eye-tightening plan?
TriLift can be useful when the eye area is being treated as part of the whole upper and midface rather than as an isolated patch of skin.
For example, a patient may have crepey lower-eye skin but also loss of cheek and muscle support. A plan could use TriLift for muscle conditioning and RF-based skin treatment while Fotona, Sofwave or CO₂ addresses the skin concern that is most visible.
This layered approach is especially useful because periorbital aging involves the skin, fat, muscles and supporting anatomy, not one structure alone.
How much downtime should I expect?
Downtime depends heavily on the technology and intensity.
Sofwave and Fotona SmoothEye are generally selected when minimal interruption to daily activities is important. Fotona describes SmoothEye as non-invasive, while the published Er study reported short-term redness, swelling and superficial peeling.
Fractional CO₂ is a resurfacing treatment and normally requires more recovery. RF treatment recovery varies by whether the treatment is non-invasive or uses microneedles.
The more aggressive option is not automatically the better one. Treatment should balance the desired degree of improvement against recovery, skin type and risk of pigment change.
Is laser treatment around the eyes safe?
The eye area requires specific precautions. Health Canada notes that cosmetic lasers can cause burns, pigment changes, scarring and eye injury if the wrong device or technique is used, and specifically advises appropriate eye protection and treatment by properly trained professionals.
Published reviews of periocular laser complications also emphasize the importance of appropriate eye protection because ocular injuries, although uncommon, can be serious.
For that reason, treatment around the eyelids should follow device-specific protocols and should never be approached like an ordinary facial treatment zone.
When is a non-surgical eyelid lift not enough?
Non-surgical treatments make the most sense when the problem is mild to moderate skin laxity, fine wrinkles, crepey texture or early brow descent.
If there is a large amount of redundant upper-eyelid skin, a lid margin that is truly drooping, functional vision changes or advanced tissue descent, a surgical assessment may be more appropriate. Energy devices can improve skin quality, but they do not physically remove a large fold of excess tissue.
The goal is not to force every eye into a non-surgical treatment plan. It is to identify what is making the eye area look heavy or aged, then choose the least invasive approach that can reasonably address it.
For some patients that may be one Sofwave treatment plan. For others it may be a Fotona series, CO₂ resurfacing, RF, TriLift or a staged combination.
Results vary from person to person and are not guaranteed. Nothing here is medical advice — book a consultation and we will look at your case.
